Finding Help at Home and Moving to Residential Care

Whether your person is living at home or when the time comes to move to a residential facility, important conversations and decisions need to be made. Join MaryLou Hernandez, LCSW, Family & Community Services Social Worker, for an informative discussion that offers a variety of resources to help families facilitate a smooth transition when faced with either decision.

Help at Home: Support for the person living at home (9:30 a.m. - 10:45 a.m.)

Residential Care: When moving becomes the best option (11:00 a.m. - 12:15 p.m.)

A 15-minute break will be held between the two sessions. Come to one or both sessions.
